Description

Led by trained facilitators, participants are encouraged to explore and identify their goals and values for future healthcare decision making.

Advance care planning is a process that helps you:

  • Choose a healthcare agent.
  • Reflect on your values.
  • Explore goals for treatment.
  • Take the first steps toward completing an advance directive


This is an interactive advance care planning workshop and will focus on completing your advance directive document with the support of class instructors available to answer your questions. All participants will be guided through the process of choosing a healthcare agent and putting your wishes in writing related to your future health care in the event you are unable to speak for yourself.
