Audience

We welcome all nurses, physicians, social workers, physical therapists, occupational therapists, and individuals working with the older adult.


Details

This program will provide current information and evidence-based practice guidelines that will enhance clinical and professional practice related to the care of the older adult. All of the interdisciplinary speakers will provide insights on implementing the 4Ms Framework. Topics will include enhancing knowledge of patient priorities care, pharmacology of cannabinoids, research around physical activity, delirium prevention tools, and much more!

 

Program Objectives:

  • Discuss integrating Age-Friendly care across the continuum
  • Appreciate fundamental Patient Priorities Care (PPC) as a preferred framework for eliciting and aligning care with the What Matters 'M' of Age-Friendly Care
  • Be comfortable using patients' health priorities as the focus of communication and decision-making about their care
  • Identify the current legal status of medical marijuana and other cannabinoids
  • Describe the clinical pharmacology of cannabinoids
  • Describe recent research on physical activity levels of hospitalized patients
  • Identify Social Determinants of Health (SDOH) and how these impact health outcomes

  • Identify possible community-based services to support older adults and their caregivers for patients who are discharging back to the community

  • Identify evidence based practices to prevent and manage delirium

 

In order to receive continuing education hours for this event, you must meet the following completion requirements:

  • Attendance at the entire activity
