We welcome all nurses, physicians, social workers, physical therapists, occupational therapists, and individuals working with the older adult.
This program will provide current information and evidence-based practice guidelines that will enhance clinical and professional practice related to the care of the older adult. All of the interdisciplinary speakers will provide insights on implementing the 4Ms Framework. Topics will include enhancing knowledge of patient priorities care, pharmacology of cannabinoids, research around physical activity, delirium prevention tools, and much more!
Program Objectives:
Identify Social Determinants of Health (SDOH) and how these impact health outcomes
Identify possible community-based services to support older adults and their caregivers for patients who are discharging back to the community
Identify evidence based practices to prevent and manage delirium
