The letter must be completed by the sponsoring dentist. Preferably printed on letterhead, the letter verifies:
*Experience may be substituted with successful completion of a dental assisting program. If experience is substituted, kindly provide a copy of completion certificate with along with the recommendation letter.
Please note: Attendees MUST be currently working in a dental office to participate in this course. No exceptions.
This course is designed for dental assistants who would like advanced training in restorative dentistry. The course will provide pre-clinical proficiency in restorative procedures and will include placement and finishing of amalgam and composite restorations to clinical competency. The course consists of both Didactic and Practical components presented in two weekend sessions with an “intersession” in the participant's office of employment.
Please note the following:
1. Didactic: Lecture: This will be verified by written exam.
2. Intersession: Employer's evaluation of restorative ability (chairmount and dentoform) supplied by ULSD. Restorative materials provided by employer.
3. Practical: Pre-clinical: This will be verified by faculty demonstrations and evaluation of participants at the teaching institution.
Participants who successfully complete the examination and all required course requirements will receive a Certificate of Completion. Failure to pass this course will result in one (1) opportunity to re-take the entire course at a 50% reduction of current course fee within one year.
Successful completion of the course requires:
(1) A 75% pass in a multiple choice question examination as well as in the pre-clinical and clinical portions;
(2) All ULSD supplied materials must be returned at the end of the course or a $500 supply fee will be charged to the employer.
This course does not offer Dental Continuing Education but is defined as a Training Course approved by the Kentucky Board of Dentistry.
Upon completion of this course, participants should have an understanding of:
Wil M. Abshier, DMD is an assistant professor in the Department of Comprehensive Dentistry at the University of Louisville School of Dentistry. He maintains a private practice within UofL Dental Associates, is a clinical team leader for dental students, and course director for "Pain Control & Anxiety Management." Dr. Abshier received his B.S. in Healthcare Administration from Western Kentucky University; DMD from the University of Kentucky College of Dentistry; and completed a General Practice Residency at the University of Kentucky Medical Center. Professionally, he is a member of the American Dental Association, Kentucky Dental Association, Louisville Dental Society, and the American Dental Education Association.
Allison Geary, DDS is an assistant professor in the Department of Comprehensive Dentistry at the University of Louisville School of Dentistry. She maintains a private practice within UofL Dental Associates and is a faculty member for both pre-clinical and clinical instruction. Dr. Geary received her BS in Business at the Kelley School of Business at Indiana University; DDS at Indiana University School of Dentistry; and completed a General Practice Residency at the University of Kentucky Medical Center.
All University of Louisville School of Dentistry Faculty are licensed practitioners in the Commonwealth of Kentucky and have no relevant financial relationship to disclose pertaining to this course.

Cancellation Policy:
We reserve the right to cancel, modify date, course content, program faculty and activities to accommodate scheduling conflicts. Pre-registered participants will be notified if a course will not be presented should circumstances warrant such action. In the event a course is cancelled, tuition will be refunded.
All registrations are subject to the ULSD Cancellation Policy. This is a 'limited' attendance course.
ULSD must receive your written request for cancellation not later than four (4) weeks prior to course start date to process a refund. A 30% processing fee will be deducted from all refunds. Course Registrants who are a 'NO-SHOW' forfeit course tuition.
