Each individual who is serving needs a separate registration.

You will have the option to register another individual after you hit 'submit'.  

*Children under the age of 12 do not need to be registered, but must serve alongside a parent or guardian.  Except in the nursery, volunteers must be 14 years or older.

Registration Agreement 

Medical Release. In the event I cannot consent to medical treatment on my own behalf, I authorize a representative of Church of the Resurrection (Church), whether volunteer or paid, to consent to any and all recommended medical treatment in the event of an emergency. If medical treatment is needed, I agree to pay either directly or through my own insurance, all medical and hospital costs and to be solely responsible for any treatment and the cost thereof.

Waiver of Liability. In consideration of the right to participate in the activity I am registering for, and intending to be legally bound, I hereby waive, release, and agree to indemnify the Church from and against any and all rights and claims for damage, injury, accident or liability which I, my representatives, successors, and assigns may now or in the future have against The United Methodist Church of the Resurrection, its staff, governance members, volunteers, and affiliates arising out of or relating to the activity. I acknowledge that I am participating at my own risk. I agree to assume full financial responsibility for any damage or injury I may cause to the Church or others.  *This does not apply to persons employed by Church of the Resurrection. 

Photo Notice. The Church occasionally takes photos or videos of its activities, programs, and events. By registering for this activity, I consent to the use of my likeness in photographs, videos, or voice recording by the Church for promotional and educational purposes, whether nonprofit or commercial. I waive any right to inspect or approve the use of my likeness and any compensation for such usage. To opt out of this consent, I understand I can contact the location person in charge (will receive name and contact info in confirmation email). When photos or videos are used, participants will not be identified by name.
