Guidelines and Confidentiality Agreement for Shadowing and Observing Experiences

Confidentiality

  • All information will be kept confidential, except when asked to share information as appropriate, to those who “need to know” in order to perform their job duties.
  • Access to and use of confidential information in verbal, written or electronic form is a privilege. Access and use of confidential information will be limited to what is necessary to perform job duties.
  • If there is a business need to know, any discussions of confidential information must be made where others including co-workers, patients and visitors will not overhear.
  • Breach of confidentiality is not limited to verbal communication. It includes all types of written and electronic communication including but not limited to blogging, emails, and posting on social media sites.
  • Noncompliance with confidentiality is a breach of HIPAA punishable by law and may be subject to prosecution. If found guilty, penalties may include jail time and/or monetary fines.
  • Any breach of confidentiality will result in immediate termination of educational privileges and possible prosecution.

What information is confidential?  

Examples of confidential information include, but are not limited to:

  • Individually identifiable patient information in any form, including name, address, demographic information, clinical history, results, billing data and appointment schedules and dates of service, or any other information that could identify a patient.
  • Business information regarding TRIA Orthopaedic Center including financial, payroll and personnel information concerning employees of TRIA Orthopaedic Center.

Dress Code/Personal Appearance

It is the expectation that you will dress in such a fashion as to create a positive impression on patients, customers, visitors and others. Radical departure from expected dress or grooming will result in termination of educational privileges.

Dress Code Guidelines:

  • All clothing, including shoes, must be clean and in good repair.
  • Clothing should not be transparent, revealing or tight fitting, must cover the upper leg and midriff area, and not interfere with performing job functions.
  • Printed t-shirts, sweatshirts and caps with logos or advertising are not acceptable.
  • Hose/stockings should be worn at all times.
  • Hair must be clean and neat. Extreme styles and unusual colors are prohibited.
  • Capri pants, shorts or knee-length pants, yoga pants, jeans, and leggings are not acceptable.
  • Jewelry must be limited for safety reasons and no exposed pierced jewelry, other than earrings, are allowed.
  • Makeup is to be worn in moderation.
  • Perfume/cologne is prohibited due to the prevalence of sensitivities.
  • Mustaches and beards are to be kept neat and well groomed.

Harassment and Offensive Behavior

TRIA Orthopaedic Center provides a safe, healthful and productive work environment. Such an environment must be free from intimidation, hostility or other offenses that interfere with work. Harassment will not be tolerated and will result in immediate termination of educational privileges. Harassment, which is based on an individual’s race, color, creed, religion, national origin, sex, disability, age, marital status, sexual orientation or status with regard to public assistance, is expressly prohibited.

What is harassment or offensive behavior?

  • It can take many forms. It may be, but is not limited to words, signs, jokes, pranks, intimidation, physical contact, violence or behaviors intended to demean or belittle another individual.
  • Sexual harassment, may include unwelcome sexual advances, requests for sexual favors, or other verbal or physical conduct of a sexual nature when such a conduct creates an intimidating, hostile or offensive work environment, prevents an individual from effectively performing the duties of their position, or when such conduct is made a condition of employment or compensation, either implicitly or explicitly.

 

Workplace Violence

  • Violence is defined as the threatened or actual use of physical force against a person or group that results in or is likely to result in emotional damage, injury or death.
  • Violence can be either real or implied.
  • Violence includes, but is not limited to, verbal or physical threats, carrying a potentially dangerous weapon, acts of physical assault, following or stalking, threatening telephone calls or letters, verbal and non-verbal threats, and vandalism.
  • TRIA Orthopaedic Center has a zero-tolerance policy for workplace violence resulting in immediate termination.

 

Additional

  • If I am injured or become ill on TRIA Orthopaedic Center property as an observer, any medical bills related to treatment of the injury or illness are solely my responsibility.
  • I have read and understand the information given to me and agree to comply with all TRIA policies and procedures. Failure to comply will result in termination of educational privileges which will not be granted again in the future.
 I do NOT authorize Park Nicollet Health Services (PNHS) and/or a related organization to create, use and disclose photographs, video recording, audio recording or other recording.
